Contacting Us By Phone

On the Phone

If you have an emergency, call 911 immediately or go to your nearest emergency room. Do not leave a message on our voicemail.

 

In order to handle our telephone communications most efficiently, let us share some basic information about our telephone procedures:

After Hours, Holiday and Weekend Calls

Our physicians are on call 24 hours a day, 7 days a week. Please call the main office number. The answering service will contact the physician on call if the office is closed.  

 

Non-Emergency Calls should be made between 8:00a.m. and 5:00p.m. Monday thru Thursday and between 8:00 a.m. and 3:45p.m. on Friday.

One of our well-trained and experienced triage nurses will most likely take your call and answer any questions she can, When needed the nurse will check with the physicians.

If our nurses are busy, you will be asked to leave a message on our nurse line voice mail, option 4. This voicemail box is checked frequently throughout the day and messages/calls are returned usually within 24 hrs or less. Our Nurses try to call you and let you know they received your message and are working on it.

Directions to Our Office

From the North:

  • Follow 75 South

  • Exit the highway at Exit 50B, SR 741 South, Moraine/Kettering

  • At the end of the ramp, turn left onto N. Springboro Pike.

  • Follow N. Springboro Pike  3/4 of a mile and turn left on W. Dorothy Lane.

  • Follow W. Dorothy Lane for 2.5 miles.

  • Turn right on Far Hills Ave.

  • Follow Far Hills Ave for 2.5 miles.

  • Turn right on W. Rahn Rd.

  • Turn right into the Frank Lloyd Wright Building, the first driveway on the right.

 

From the East:

  • Follow 70 West to 675 South

  • Follow 675 South to Exit 4, SR 48, Centerville/Kettering

  • At end of ramp, turn right onto Far Hills Ave.

  • Follow Far Hills Ave 1 3/4 miles to W. Rahn Rd.

  • Turn left on W. Rahn Rd.

  • Turn right into the Frank Lloyd Wright Building, the first driveway on the right.

 

From the South:

  • Follow 75 North to 675 North.

  • Follow 675 North to Exit 4B, SR 48, Kettering.

  • Merge onto Far Hills Ave.

  • Follow Far Hills Ave 1 3/4 miles to W. Rahn Rd.

  • Turn left on W. Rahn Rd.

  • Turn right into the Frank Lloyd Wright Building, the first driveway on the right.

How do I Pay My Bill Online?

Credit Cards

Avoid overdue bills and late charges by paying your bill online. Use your credit card to pay your balance and keep your account current.

Click here to pay your bill online. If this is your first time using www.payyourdoctor.com, click ‘Create an Account’ on the right side of the screen. If you are a returning user, simply log in.

This only allows you to pay your bill; you will not be able to view outstanding balances, invoices or payment history.

Instructions for Paying your Bill Online

  • Verify your demographics.

  • Enter your account number and payment amount.

  • Make your payment.

  • Print out your confirmation screen and maintain it for your records.

How Do I Receive My Lab Results?

We do not notify patients of test and lab results if they are normal.

 

You will be called when tests are reported to be significantly abnormal.

 

Please ensure we have your current address and phone number(s).

 

You are more than welcome to call our office or message us through the patient portal for your results. You may leave a voicemail on our nurse line and we will call you as we are available.

 

If you choose to message us through the patient portal, we can answer you in your message box and send copies of test results, if requested.

How Do I Obtain a User ID and Password for the Patient Portal (Follow My Health)?

To register as a user for our patient portal, please call the office at

(937) 433-8990.

 

We will send you an invite to register for a portal account.

Please make sure to remember your user ID and Password for later sessions.

 

For security purposes, the office cannot give out User IDs over the phone. If you have forgotten your password and remember your user ID you can click on the “forgot my password” button located on the log in screen.

Patients are seen by appointment only.

We know that your time is as valuable as ours and we make every effort to honor all of our commitments.

 

You can help by arriving at least 15 minutes prior to your appointment for completing/updating paperwork or you can complete your registration paperwork by clicking here.

This will take you to the patient portal.

 

We request that you call our office at least 24 hours in advance to cancel or reschedule an appointment or you may be subject to an administration fee.

 

Occasionally, unexpected delays and emergencies arise which may increase your waiting time. Be assured we will make every effort to see you as scheduled.

 

If waiting will be an inconvenience, please see the receptionist to reschedule your appointment.

How Do I Request a Prescription Refill?

Click here to log into the patient portal and request a prescription refill.

 

If you do not have a portal account, please call (937) 433-8990, option 4 between 8:00 a.m. and 5:00 p.m. Monday thru Thursday and between 8:00 a.m. and 4:00 p.m. on Friday.

A nurse will review your chart to determine if a prescription refill is the best course of action. 

​

Your request will be reviewed and processed during normal business hours. If you request a refill after hours or on weekends, it will be reviewed and processed on the next business day. Please request renewals at least 72 hours in advance. Please have the pharmacy name and phone number available when you call.

​

If you have not been seen by a physician at SDACC in the past 12 months, your request may be denied, and you will be directed to schedule an appointment.
